Why do icon images not always display in the Media Type column when viewing the Borrowing Requests New for Review list?

Answer

Icon images don't display - or don't display consistently - under the "Media type" column of the Borrowing requests: New For Review list.

In your ILL request form, "Format" must be an option. If the field is not required, the icon will only appear if the patron chooses to edit that field when submitting a request using the form. If the patron does not make a selection, then that information is not coming through on the initial request. 

Adding "Format" as a required field in your request form should fix this.
