Are my notifications sent according to how I set them in Service Configuration, or at the top of the hour/the next morning?

Answer
The setting in your Notification Policy specifies when the notification is created and "sent" to us. Then, we send it along with all other notifications in our queue to the patrons in batch at the top of the hour or the next day, depending on the kind of notification.
If the item changes status or another notification is triggered before the batch sending, the notification will not be sent.
